Community Health Survey: Learning to Communicate Effectively with the Community



The Chicagoland Asthma Network (CAN), the CAC's Community Task Force, recently created a Community Health Survey to learn how to best deliver health information to community residents. It asks questions about what, where, and through whom the community would like to receive information on health topics.

Please help us obtain feedback from community residents! Our goal is to collect 1,000 surveys. Survey instructions as well as the surveys in both English and Spanish can be found here. Please print, distribute, and return the surveys by September 30, 2008.

The very important results of the Community Health Survey will be used to create a plan of action to reduce asthma in Chicago from the grassroots level upwards. The results will be posted on the CAC website, incorporated into the upcoming fall and spring CAC meetings on healthcare disparities, and shared with all community organizations and individuals that help in data collection.
