Instructions for Rating a Clinic

Please take a few moments to rate a clinic that you have visited in the last 30 days. The evaluation will take less than 5 minutes to complete. To begin, find your clinic by Searching by Address or by Clinic Name. Then click "Rate Clinic" as show below:

Then complete the survey developed for and by community members.

This section of our website, intended to help community members select medical providers that best matches their needs, contains information for more than 200 clinics in Chicago. By rating the clinics we hope to improve healthcare services by letting community members know which clinics are providing the best services. Only ratings provided by clinic patients will be accepted. If you are aware of rating being conducted by individuals other than clinic patients, please contact us immediately.

All responses are completely anonymous. Your information will NEVER be shared. Once you have completed your rating it will immediately be posted on the website.
